Consent and Health Declaration
Please read this in full before booking. It is here to keep you safe and to make sure you know what you are saying yes to. When you book, you will be asked to confirm that you have read and agreed to it. If anything is unclear, or you would like to talk something through, please contact me before you book.
1. Understanding the practice
Groundwork breathwork sessions, online and in person, are facilitated by [your full legal name].
Conscious connected breathwork involves sustained, active breathing. It can bring up strong physical sensations, such as tingling, temperature changes, muscle tension or lightheadedness, and it can bring up strong emotions. This is a normal part of the practice.
Taking part is voluntary. You can slow down, pause, or stop at any point, and you do not need to explain why. You are responsible for listening to your own body and for adjusting or stopping if you need to.
2. Health and wellbeing declaration
Breathwork is not suitable for everyone. Please read this list carefully.
Breathwork is not recommended if you have any of the following:
Cardiovascular conditions, including high or abnormal blood pressure, a history of aneurysm, heart attack or stroke, or heart disease
Uncontrolled thyroid conditions
Uncontrolled diabetes
Epilepsy or a history of seizures
Severe asthma
Glaucoma or a detached retina
Osteoporosis
Recent surgery
Diagnosed severe PTSD
Severe mental health conditions such as psychosis, schizophrenia or bipolar disorder
Any mental health concern that is not currently supported through treatment or a support network
Being in the middle of an acute emotional or spiritual crisis
Pregnancy
This list is not exhaustive. If you have a health condition that is not named here and you are unsure whether breathwork is right for you, please speak to your doctor before taking part, and feel free to contact me too.
3. If something on that list applies to you
If you recognised yourself in the list above and you would still like to explore breathwork, please get in touch with me directly before booking.
A group session will not be the right setting for you, and that is not a door closing. It means we take more care. We can look at a tailored one to one session, shaped around your needs and what is safe for you, so that you can still receive the benefits of the breath in a way that is held properly.

4. Your session environment
For online sessions, please make sure you:
Are in a private, comfortable space where you can sit or lie down
Are not driving or operating machinery
Have water within reach
Where possible, have someone you can contact if you need support afterwards
For in-person sessions, please let me know if you feel unwell, unsafe, or overwhelmed at any point. You can leave the space at any time.
5. Assumption of risk and release
Breathwork carries inherent risks, and by taking part you accept responsibility for your own participation.
To the fullest extent permitted by law, you release Groundwork and [your full legal name], along with any affiliates, contractors, employees or volunteers, from liability for any injury, illness or loss arising from your participation, except where it is caused by their negligence. You agree not to bring a claim in respect of the inherent risks of breathwork that you have knowingly accepted.
(Note: under UK law you cannot sign away your right to claim for death or personal injury caused by someone's negligence. This clause reflects that.)
6. Your agreement
When you book a Groundwork session, you will be asked to confirm the following:
You have read and understood this document
To the best of your knowledge, none of the listed health conditions apply to you, or, if one does, you have contacted me directly first
You are taking part voluntarily and accept the inherent risks of Breathwork
Your booking, with your name, email and the date, is kept as the record of your agreement.
